Transform Your Resolutions with the Conscious Change Model: Embodying Your Future Self

As we move further into 2025, many of us are already navigating the intentions we set for the year. Yet, by now, you may have noticed that sticking to resolutions is often more challenging than expected. This isn’t due to a lack of motivation—it’s because many resolutions focus solely on external goals without addressing the deeper layers of transformation required to create lasting change.
At ALCN, we teach that true transformation isn’t just about what you do—it’s about who you become. The Conscious Change Model, combined with NLP Somatic Coaching tools, provides a powerful framework for aligning your goals with the seven levels of change. This approach moves beyond surface-level resolutions, guiding you toward an embodied, lasting shift.

Where Are You Now? Mapping Your Current Reality

The Conscious Change Model, inspired by Robert Dilts’ Neurological Levels, invites you to reflect across seven interconnected levels of change. Combined with our Embodiment Model, it asks: Who do you need to become to align with your vision?

  1. Environment: What’s going on in your physical environment—at home or work? Does it support the life you want to create? Who in your friendship or professional network understands and champions your vision for the year ahead?
  2. Behaviors: What habits, behaviors, and patterns are you engaging in daily? Are they supporting or sabotaging the changes you want to make?
  3. Skills: What are your natural superpowers? What new skills, tools, or knowledge do you need to cultivate to support your desired outcomes for this year?
  4. Beliefs: What do you believe is possible for you? Are there limiting beliefs holding you back from fully stepping into your potential?
  5. Identity: How would you describe yourself today? What roles do you play in your own life and the lives of others? How does this align with who you want to become?
  6. Mission/Impact: What is the deeper “why” driving your actions? How connected do you feel to your larger purpose and the impact you wish to make?
  7. Vision: What is the bigger picture for your life? What does success look like to you at the end of 2025?
Take a moment to reflect on these questions. Which levels feel aligned, and where do you notice resistance or disconnection?

Who Is Your Future Self? Envisioning the End of 2025

Now, fast-forward to December 31, 2025. Picture the most empowered, aligned version of yourself—the one who has not only achieved their goals but also embodied their desired changes.

  • Where is your future self physically and emotionally? Who are their support team and champions?
  • What habits and behaviors have they mastered?
  • What new skills or capabilities have they cultivated to sustain growth?
  • What beliefs have supported their transformation?
  • Who are they at their core? What identity have they claimed and embodied?
  • What purpose drives their daily actions? What impact are they making in the world?
  • What is their vision for the future they are helping to create?
Visualization is a powerful NLP tool for creating clarity and aligning action with intention. As you envision your future self, notice how it feels in your body. Embodiment creates a direct pathway for transformation, helping you step into this version of yourself today.

What Needs to Shift?

To close the gap between your current reality and your future self, consider what needs to change across the seven levels:

  1. Environment: What adjustments can you make to your space or relationships to create a supportive environment?
  2. Behaviors: What small, consistent daily habits can you adopt to align with your vision?
  3. Skills: What new tools or knowledge will help you become the person you aspire to be?
  4. Beliefs: Which limiting beliefs need to be released? What empowering beliefs can replace them?
  5. Identity: Who do you need to become to embody your vision fully?
  6. Purpose: How can you stay connected to your deeper “why” when faced with challenges?
  7. Vision: How can you hold a clear, inspiring vision while remaining flexible and open to change?
NLP techniques like reframing and anchoring, combined with somatic practices such as grounding, breathwork, or mindful movement, help you embody these shifts, making change a felt experience rather than just a mental exercise.

Embodying Your Future Self Today

One of the most transformative aspects of the Conscious Change Model is the focus on embodiment. Your future self isn’t a distant reality—it’s a way of being you can choose right now. Ask yourself:

  • How can I embody my future self today?
  • What daily actions, postures, or mindsets will align me with my vision?
Somatic tools provide practical ways to anchor your future self in the present moment, helping you create a felt sense of possibility that drives consistent, intentional action.

The Journey Ahead

Your resolutions for 2025 are not just tasks to complete—they’re opportunities to become the most aligned, empowered version of yourself.

Take a moment to map out your seven levels of change. Reflect on these questions:

  • What is one shift I can make this week to align with my vision?
  • Who in my life can support me on this journey of transformation?
Your future self is not a dream—it’s the embodiment of the intentional steps you take today. Let’s make 2025 the year you align with your highest potential and create conscious, lasting change.
